EXPENSE FORMS

To receive Reimbursement for Club expenses you have paid for, please complete the Expense Voucher and send it to the Treasurer.  Please note that this is a fillable PDF Form.  You can enter the information, download and save the form and attach it to an email if you wish.  Please email shahlabutler@comcast.net.

Please complete Donation Voucher if you donate items to the Club and want a receipt for your tax deductions.

If you have made any purchases using our Club’s Tax Exempt Status, you are requested to please fill out this Tax-exempt Usage Report and submit it to our Treasurer for filing.

Use this form if you need to provide information regarding our tax-exempt status.

In addition to providing grants to organizations around Calvert County, members can request that a donation be made to a charity of their choice,  The Charity Donation Form needs to be filled and sent to the Grant Committee for consideration.
