EXPENSE FORMS

To receive Reimbursement for Club expenses you have paid for, please complete the Expense Voucher and send it to Treasurer.  Please note that this is a fillable PDF Form.  You can enter the information, download and save the form and attach it to an email if you wish.  Please email to shahlabutler@comcast.net.

Please complete Donation Voucher if you donate items to the Club and want a receipt for your tax deductions.

If you have made any purchases using our Club’s Tax Exempt Status, you are requested to please fill out this form and submit to our Treasurer for filing.

In addition to providing grants to organizations around Calvert County, members can request that a donation be made to a charity of their choice,  The Charity Donation Form needs to be filled and sent to the Grant Committee for consideration.
